Formulary and RAG submissions

The criteria for consideration of a drug can be found below. If your request fits at least one of the criteria for consideration, then please download and complete a copy of form 1 – new medicine request form. This form should be used to:

• add a new drug to formulary, or a new indication for an existing drug
• amend the formulary, such as changing a drug from alternative to first choice or remove a drug
• for minor amendments, such as correcting an error or adding links to safety alerts or guidance,
• to amend a RAG status

Please note that forms must be completed by a healthcare professional currently working in the NHS in Greater Manchester, and supported and signed by a clinical / medical director or head of medicines management. Ideally these should also have been considered and approved by the local drugs and therapeutics committee prior to submission to the Clinical Reference Group.
